To fill a new prescription using Home Delivery:
1.
First, have your doctor write a prescription for up to a 90-day supply of your medication. The prescription should include refills for up to one year, if appropriate.
2.Next, print a prescription order form found here.
3.Lastly, submit your order.
Note: If you have a credit card or check card on file with Express Scripts, your card will be billed automatically.

Fax (Doctor Only)
Have your doctor or your doctor's staff fax your completed order form to 1-800-837-0959. Faxes must be sent from your doctor's office. Faxes from your home or workplace cannot be accepted.
Online/Mobile App
Log in to express-scripts.com or the Express Scripts mobile app, choose the medicine you want delivered, add it to your cart, then check out.
Phone
Call Express Scripts at the toll free number on the back of your ID card for assistance in switching to home delivery.
Order by Mail
Mail your completed order form and your written prescription to the address below. Please write the patient's name, ID number, address and date of birth on each prescription.

After receiving your order, Express Scripts will process and ship the order in three to five business days. Please allow additional time for postal service delivery. For your protection, Express Scripts requires a doctor's signature on all prescriptions.
