There are occasions when you will need to update your PSSU Health & Welfare Fund information.
If you've recently:
· been married or divorced,
· added a son or daughter,
· moved to a new address,
· changed your name,
or your child is no longer eligible for benefits,
then you will need to fill out a new Census Card. Please click on the update link above or call or email us if any of the above changes have taken place so that we can send you the necessary forms, cards, etc.
About your full-time student age 19 or older: If your child has had his/her 19th birthday, he/she is no longer eligible for benefits, unless he/she is a full-time student. The benefits for your child will continue if you have filled out the Student Certification Form and had it stamped with the school seal (or have the school attach their own version of verification) and mail the form to our office. Note: Please do not send acceptance letters, class schedules or bills. These documents are not acceptable as proof of attendance. We will then notify the benefit providers that your student is eligible for benefits. Please note that your child's name may not appear on your Express Scripts or NVA card. Your child IS eligible for benefits, but the name may not be printed on the card! Download the Student Certification Form HERE.
This form must be filled out twice/year at the beginning of the fall and spring semester of school!! Fall semester form must be received by September 1st, and Spring semester by January 1st. Benefits will be terminated retro to January 1 if the spring semester form is not received by January 1st and retro to July 1 if the fall semester form is not received by September 1st if your student's form is not received by these dates.
